[Bug] Claude Code executes changes without confirming intent or answering user questions first

Bug Description
multiple times I have asked a very simple question of Fable and it has gone off and started making changes without answering the question in the first place and definitely without clarifying whether I want it to actually DO anything. super frustrating, wastes tons of time and tokens, and could be dangerous
