[BUG] Claude code requests permission for every change made in .claude/commands

What's Wrong?

When using claude code to improve commands in .claude/commands folder, every single change needs to be approved separately:

Do you want to make this edit to create_plan.md?
❯ 1. Yes

  1. Yes, allow all edits during this session (shift+tab)
  2. No, and tell Claude what to do differently (esc)

What Should Happen?

When selecting " 2. Yes, allow all edits during this session (shift+tab)", we it should continue without asking again.

  1. ask to update a command in .claude/commands
  2. allow changes
  3. it still keeps asking
