[Bug] Fable 5+ consistently ignores defined skills and creates incorrect memory workarounds

Bug Description
Fable 5+ will NOT use skills correctly. EVERY time, insists on ignoring or deviating from skills and then writing new "memory" to "correct". 100% failure rate across 5 worktrees with 10x repeats each. Literally lost 20M+ tokens this week alone.
