Incorrect Custom Slash Command Prefix for User-Defined Commands

Bug Description
I have several user-defined custom slash commands in ~/.config/claude/commnds. They are recognied and I can use them in the prompt. The issue is that I expect the commands are prefixed with "/user:command-file-name", but what I get is "/command-file-name". This is inconsisten with the document site at https://docs.anthropic.com/en/docs/claude-code/slash-commands#custom-slash-commands.
