[BUG] Chat Title from VS code claude code does not sync to desktop claude code.

What's Wrong?

I am using claude code desktop app for explore and plan phases of my app and VS code (claude code) to build the app. I asked agents to put the chats under same project. I see the VS code conversation in the claude code desktop app and is intact. However, I renamed the chat in VS code and name in desktop app did not get updated, though the conversation is up to date.

What Should Happen?

I expect the chat name in desktop app to sync with vscode

Steps to Reproduce

1. Create a project in claude dode desktop app. start conversation under it.
2. open VS code, use the project and create a new chat. Remore is enabled on VS code.
3. in desktop app, you see the second chat that was created and you can see the same conversation as in VS code
4. Rename chat in VS code.
5. check the name of the VS code chat in desktop app
