[Bug] Opus 5 responses frequently end with "Two things..." phrase

Bug Description

Opus 5 ends with "Two things ..." obnoxiously often

At the end of nearly any work, Opus 5 will tell me "two things you should be aware of" or "two things that you should follow up on" or "two things that I should put in front of you" or ... you get the idea. The repetition passes through wearying into annoying; at some point, it starts to feel like Claude is just trying to get two more pieces of work out of me with no consideration of whether the items are a valuable use of my time and focus or its tokens.

The only thing I can say in its favor on this is that I called it out on this in a particularly long session where every response contained this pattern and its response got a laugh out of me: "Fair. Next time I'll make it three."
