claude-in-chrome extension will not connect to a Claude Code / Cowork session. All `tabs_context_mcp` calls return "Browser extension is not connected" regardless of troubleshooting.

Steps taken (none resolved it)
- Restarted Chrome (quit and relaunched, not just closed window)
- Toggled the extension off/on in chrome://extensions
- Confirmed chrome://extensions shows no error state on the extension
- Restarted Claude session
- Restarted Chrome and Claude together
Connection still fails after every attempt above.
Expected behavior
Extension reconnects after a Chrome/extension restart and tabs_context_mcp succeeds.
Actual behavior
Every connection attempt returns:
Browser extension is not connected. Please ensure the Claude browser extension is installed and running (https://claude.ai/chrome), and that you are logged into claude.ai with the same account as Claude Code.

Additional notes
Was working earlier in the same session, then dropped mid-conversation with no user action that should have disconnected it. Has not recovered since.
