[Bug] Model auto-implements changes without explicit user approval after clarification requests

Bug Description
even though it's a light matter, this has happened way too many times - I ask for something for my decision, model presents, I ask for clarification, model does changes and automatically decides and goes on with implementation. This should not be acceptable behavior and has happened a lot more with Opus 5 than with 4.8/4.7.
