[Bug] Claude Code implements plan instead of updating plan when requested

Bug Description
Claude Code keeps jumping the gun, and implementing the plan. My prompt was to UPDATE THE PLAN. I realize "it's eager", but that's not what I asked it to do.

When asked, it claims that I told it to "Implement the plan", which I didn't. I told it to update the plan on disk (e.g. written to disk).
