[Bug] Plan mode stuck in loop when updating file to empty content with post tool

Bug Description
I'm trying the plan mode overall and I've asked to update a specification file to be returned to an empty file. That just doesn't seem to work. It's getting stuck, getting in the loop using Opus 4.5 and it's really frustrating.

I think it's the current version which is causing this because I've been doing this quite a lot until the latest 2.1.12 came in, right? So yeah frustrated overall. One more input if that helps. It's stuck on using the post tool. It says 0 of 3 done and never completing at all.
