[BUG] Scheduled CCR routine occasionally starts sandbox but never delivers initial prompt (hangs indefinitely)

ルーティン(Trigger) ID: trig_01Anc3RdUaxkAyPMqUnyhoMF
症状: cron発火でサンドボックス・Claude Codeプロセスは起動するが、初期メッセージ（ジョブのプロンプト）が配信されず、worker_status: idleのまま何時間も停止する。手動でセッションを開いてメッセージを送ると初めて動き出す。
再現セッション:
cse_01SaA6nDJb4wmMj1BNVbpjBa（2026-08-13 22:16 UTC発火、翌12:02までメッセージなし）
cse_01DiesasmvoQH6yuCCt2D942（2026-08-14 12:08 UTC、手動run実行だが同様に停止）
cse_01E5ZTYSeRGB3U3to4bAX7rF（2026-08-15 07:07 JST発火、現在も停止中）
頻度: 直近数日で3回発生（正常完了する日もある＝間欠的）
