Bug Description
Ultra Review attempt counted despite producing no result
I ran Ultra Review (1/3), but it failed with the following error:
<remote-review>{"error": "Review failed: all review agents terminated before completing. See session logs for details."}</remote-review>
When I retry, Claude offers Ultra Review 2/3, meaning the failed attempt was counted against my allowance even though it produced no review. Failed attempts that return no result should not consume an Ultra Review credit.
