[BUG] Plan file intermittently reported as non-existent while composing inline comments; unsaved comment text is lost

What's Wrong?

While in plan mode, composing an inline comment on a section of the plan, the UI intermittently reports that no plan exists. Reopening the plan restores it, but everything typed into the comment box is gone and has to be retyped.

Steps to reproduce

Enter plan mode; let the assistant write a plan file.
Select a passage in the plan and begin typing an inline comment.
Intermittently — partway through typing — the UI reports no plan exists.
Reopen the plan. The plan itself is intact; the in-progress comment text is lost.
Expected: in-progress comment text survives, or is restored as a draft when the plan reloads.

Actual: the comment box is cleared and the text is unrecoverable.

Impact: loses user-authored text with no recovery path. Worse for long comments, which is exactly when it hurts most.

Claude Code, desktop app on Windows 11 Home 10.0.26200
Model: Opus 5
Frequency: intermittent, several times in one long plan-mode session
Plan file location: ~/.claude/plans/<name>.md
