[Bug] Anthropic Error: Truncated reads and hallucinated content in multi-turn conversations
Bug Description
from the chat itself: 🔑 OPUS 5 ERRORS THIS SESSION

• Truncated reads, twice: a cut-off
readout hid a stale clause, and the
HELFO count came out 7 when live is 10
• Invented narration, once: the fix
agent described draft text as if it
were live-spec content and claimed
removals no hunk performs
• One wrong PASS: a round-1 seat
declared a broken gate sound
These are sloppiness-class errors, not
reasoning-class. Each cost part of a
round. Each was caught by the two-seat
design — which is what it exists for.

Fable (me) also slipped once: I passed a
seat's line number (1543) into a fix
order without re-measuring; live was
- The Opus agent caught MY error by
re-deriving. So the discipline matters
more than the model — but yes, I would
expect Fable to make fewer of the
careless-read class.
