[Feature Request] Add built-in accessibility output style with answer-first formatting and complete sentences

Bug Description
Accessibility feedback: readable output should not require this much engineering.

I have a reading disability. Dense prose and compressed fragments are both unreadable to me. I need short, numbered, complete sentences with the answer first.

Getting Claude Code to produce that took me weeks of failed CLAUDE.md iterations, then a full working session with Claude to discover output styles, write a custom style, fix rule conflicts, and patch new failure modes (fragment compression, restarted numbering) one at a time. I am a power user. Most people with reading disabilities would have given up.

Specific problems:

1. CLAUDE.md formatting instructions decay over long sessions, and nothing in the product tells you that or points you to output styles as the durable layer.

1. The /output-style command failed with "Unknown command" on my install and I had to find the setting through /config instead.

1. When told to be brief, the model compresses sentences into fragments instead of omitting low-value content. For readers like me, fragments are as bad as walls of prose.

Request: ship a built-in accessibility-oriented output style (answer first, numbered complete sentences, blank lines between items, no fragments), surface it in onboarding, and document that CLAUDE.md is the wrong layer for formatting rules.
