[Bug] Unstable Execution: Claude Code Fails to Follow Instructions

Bug Description
claude is running like shit, not listening to commands, making random file edits, not following his markdown instructions, doing random shit. really running terribly. wasting A LOT of my time. worst expeirence i've had.
