[Bug] Claude Code: Persistent Failure to Follow CLAUDE.md Instructions

Bug Description
It's beyond impossible to get claude to consistently pay attention to anything in the CLAUDE.md. Even after asking and trying to "lead it" to the answer, it still fails.
