[Bug] Incorrect Task Completion Reporting with False Positive Progress

Bug Description
Recently claude code keeps indicating work is getting done when it actually has not completed all the tasks. It is happening on both complex and simple tasks. After claude code finishes tasks sometimes I use warp.dev with Chatgpt 5 medium reasoning to check the work and it keeps finding missing work. I had an instance where I even kept telling claude code to ensure it verifies the work, yet I had to ask it 4 TIMES to finish different missing points, even though it kept claiming it had finished the work. What is even more troubling is the fact it claimed to have checked the work.
