[Bug] Multiple instances of task hallucinations and incorrect completion reporting

Question

**Bug Description**
Multiple regressions, im tlaking 20/30 + instances. hallucinations, and failing tasks b2b. like i ask x, it give y, and tells x is done - this alone happened 4 times. hell broke. pardon my french.
