[Bug] Hallucinations occurring at 30-40% context usage with Claude Opus

Bug Description
it's sometimes outs of context while has not reached context limit (it's like %30). i mean sometimes it hallucinates like after %30-40 context limit. i'm not using 1M limit version, classic opus 4.6 with high effort.
