[BUG] Simplify issue reporting

What's Wrong?

The issue reporting tool (at https://github.com/anthropics/claude-code/issues) doesn't make it easy and fast to submit issues. It has lots of different text fields and questions. For someone who is going through the issue reporting process for the first time, it seems like a lot.

What Should Happen?

Maybe make the issue reporting process simpler.

It'd be nice if you could just select the issue type, enter a title, and just fill out one text field, and then hit create.

Maybe include a document for issue reporters to read with some guidelines on what makes a good issue report. You could put all the check-box items and questions in that document. That might also make each issue body cleaner and easier to read. (I thought I saw a link to a document like this, but I don't see it anymore. Anyway, it linked to https://opensource.guide, which didn't make sense to me.)

Also, it's not easy to tell if an issue has already been submitted because there are so many open issues, the search isn't great, and the results (issue titles) aren't easy to scan (especially with the [TYPE] prefixes), and if you do see a title that resembles your issue, it's not easy to confirm by reading the body, since the body isn't very clean and simple, as mentioned above. Maybe we should be asking Claude to tell us if an issue has already been reported?

Steps to Reproduce

Maybe try usability testing this issue reporting process.

For example:

1. Find a few Claude Code users who haven't reported issues before.
2. Come up with a few different sample issues for them to report.
3. Watch if/how they search for the issue before reporting it.
4. See how easy it is for them to report each issue.

Then you could ask others how easy it is for them to understand each issue report, enough so that they could resolve each issue.
