[BUG] Model picker shows Fable but is actually Opus, unless I manually reselect it

What's Wrong?

VS Code Claude Code extension model picker shows Fable as the select model, but when I ask the model it says it's Opus. If I first click Fable (even though it's the already selected model) it says ~Switched to claude-fable-5~ and the model says it is Fable.

What Should Happen?

The actual model should match the model it says in the picker.
