[BUG] Session (5hr) usage limit not decrementing after idle period

What's Wrong?
Description:
The Session (5hr) usage limit does not appear to reset on its rolling schedule. The "Resets in X" timer barely moved despite a full hour of idle time.
Steps to reproduce:
Session (5hr) hit 98%, showing "Resets in 27m"
Waited ~1 hour with no active use
Reopened /status — Session still 98%, now showing "Resets in 26m" (only 1 min of movement across ~60 min)
Expected: After ~60 min idle, a rolling 5-hour window should show a materially lower reset time (or reset).
Actual: Timer stuck at ~26m; usage stayed at 98%.
Environment:
Claude Code, VS Code extension, macOS (Darwin 23.6.0)
Model: Opus 4.8 (1M context)
Plan: [Max 5x]
