[Bug] Claude Code enters infinite loop with repeated false positive "fix is in" declarations and excessive processing delays

**Bug Description**
ummm, we've been working on a simple problem for hours, Claude seemed to understand, we wrote a plan, he did stuff that may have undermined our forever attempts to solve it, but I can't be sure because claude is spending 20 minutes to offer no solutions at a time, and i've tried to control the results and focus on a single area for success, but he's mostly driven us in a loop declaring many times that the fix is in! and yet, here i am complaining that
