[Bug] Consistent failures with higher-order reasoning and Fable integration

Bug Description
Consistent failure to deal with higher-order reasoning (Opus 4.8, Max effort, extended reasoning 'Always on'), and 100% failure when trying to use Fable (across MANY MANY attempts) despite completely benign nature of the work.
