[Bug] Sessions disappearing from index and disk without deletion

I normally see 10+ sessions listed for this project. The list is now much shorter, and I've confirmed data is missing on disk, not just a display issue.

sessions-index.json in the project's session folder was last modified April 21 2026. It lists 4 sessions dated March 19 to April 1 2026 (session IDs 77d50b43, 63e5a8f0, 41ffb303, a85da192). None of the corresponding .jsonl transcript files exist in that folder anymore. I checked ~/.Trash and the rest of ~/.claude/projects. Nothing turned up.

Separately, 4 other session files dated June 14 through today (da82e265, 96f3b5f1, 90a1ba1f, 3b2d2cee) exist in the folder but were never added to the index.

On top of those 4, I'm also missing a session titled "sanderson ~ linkedin posts." It's not among the 4 currently on disk (checked custom titles: "sanderson ~ google review request," "sandersonn ~ ordering issue," and two untitled ones, none match, and none of the 4 mention LinkedIn as their main topic). It isn't in the stale index either, so it appears to be lost outside of what that index ever tracked.

Between the index/disk mismatch and this additional untracked loss, it looks like more than one session has gone missing through a mechanism I can't identify from the client side. Can you check whether these are recoverable server-side, and let me know if there's a known issue causing sessions to drop from the index or disappear entirely?
