[BUG] : Claude Code CLI "Usage limit reached" while claude.ai and Claude Design work normally

What's Wrong?

Claude Code CLI displays "Usage limit reached" error and blocks all input, but claude.ai/settings/usage shows 0% on all meters. Importantly, Claude Design works perfectly on the same account.

What Should Happen?

Claude Code CLI should accept prompts normally since both Current session and Weekly limits show 0% used, and other Claude products work fine.

Steps to Reproduce

1. Open Claude Code CLI
2. Try to send any prompt
3. Immediately get "Usage limit reached" error
4. Check claude.ai/settings/usage → all usage at 0%
5. Switch to Claude Design → works normally
6. Switch to claude.ai web interface → works normally
