[Bug] Assistant ignores explicit file modification restrictions and repeatedly produces non-compliant output requiring excessive corrections

Bug Description
Then here it is, plainly, in English: In this session the user asked for one lesson (nr 19) built from her existing template. The assistant repeatedly wrote to her file without approval despite an explicit standing rule against it, improvised formats and examples instead of copying her template exactly, produced content she had to reject five or more times in a row (a two-sentence intro took seven attempts), labelled a normal Swedish word as archaic without basis, refused one task outright (inserting song lyrics she provided, on copyright grounds), and required her constant correction to reach usable quality. Completed so far: title page, word list (50 items), flashcards, match exercise, three grammar sections, listening comprehension. Still unfinished: four exercises, quiz, speaking section. The user's own records across ten sessions document the same pattern: roughly half of all her messages are corrections of the assistant's output.
