[Bug] Non-Compliant Content Filter: Legitimate Fable Project Planning Blocked

Bug Description

While using Claude Code with the Fable 5 model for project planning (auditing GitHub issues + synthesizing business PDF reports), the system automatically switched to Opus 4.8 due to a false-positive safeguards trigger, with no explanation of what caused the switch. The conversation content is product/business management (e-commerce backlog, payments, logistics) — nothing related to cybersecurity, chemistry, or any topic that would violate usage policies. This happened twice consecutively in the same session, both times after long responses generated by a multi-agent parallel workflow.

Steps to Reproduce
Model set to Fable 5
Run a Claude Code workflow with multiple parallel agents analyzing GitHub issues and internal business report PDFs
Receive the final synthesis (long response, with tables and business data)
System switches to Opus 4.8 with no prior warning or visible reason

Expected behavior
Model should remain on Fable 5 since the content doesn't touch any sensitive category (CBRN, cyber, etc.).
