[Bug] Claude Code executes tasks without explicit user permission despite configured mode settings

**Bug Description**
 1 OK, ~128 messages, 1 task in 3 variations., less tahhn 1% exection rate. acted without permission conatnatly despite mode. this is the shitties fucking chat bot.. sold as exectuion tool. recorded and saved. and this is fable 5, twice the pricve. great
