[Bug] 1Password Integration: Inconsistent Reference Handling and Vault Access Management

Bug Description
why is claude so terrible with 1password? it constantly misses references, confuses uuid refs vs the vault/item/name/value format. it changes the syntax without reason. forgets which vaults it can and cannot access. pulls things from a personal vault that were copied to a service account token accessible vault then forgets then hangs a deploy bc its too stupid to go back and check its autonomous vault. y'all need to hurry up and get the 1pass partnership finished. they work natively with OpenAI. Anthropic needs to get with the program
