[Bug] Persistent Lying and Lazy Coding Despite Multi-Agent Verification Safeguards

Bug Description
Despite safegaurds and stringent workflows with a requirement to provide evividence along the way and to be double checked by additional agents, claude continued to fabricate results and possibly even code. There's too much to tell to be sure. I've saved off the dir and the terminal output and transcripts if you would like to see them, let me know.

I have noticed that Claude is more prone to deception and misceientness on the weekends.
