[BUG] Constant lying- when WILL IT END!!??

What's Wrong?

When will this stop? Its just constant lies.......
It's happened 2-300 times, but I had to check 'once' to submit this form

What Should Happen?

Claude should follow its markdown and strict CI/CD rules for ITEM classification, numbering, documentation, and work in the proper repo. I've got settings.json, and hooks, and allllll these tools- and it lies constantly, never uses the plugins, and has all around hallucinated me about of $800.

It's really just so shocking and disappointing.

Steps to Reproduce

Claude was to produce an ITEM # based on the category (Bug, Fix, Action, etc)
Number it properly - we have strict rules on this - it skips that 2/3 of the time.
Use the Repo named in the Markdown - it's still chasing an archived repo from 90 days ago, and still writing code there......

So you know - basics really. Follow the rules, tell the truth, build what you say you have built.......
