VS Code extension: multiple recent sessions disappear on window close/reopen

Summary

When closing and reopening the VS Code window, multiple recent chat sessions are missing — not just the last one. The number of lost sessions appears to be growing worse over time (now 5 or more sessions disappear per restart).

Environment

• Editor: VS Code
• Extension: Claude Code VS Code extension

Steps to reproduce

1. Have several chat sessions with Claude Code in VS Code
2. Close the VS Code window
3. Reopen VS Code
4. Open Claude Code — recent sessions (5 or more) are gone

Expected behavior

All chat sessions should persist and be visible after reopening VS Code.

Actual behavior

5 or more of the most recent sessions disappear each time the window is closed and reopened. This is a recurring issue that appears to be getting worse (previously fewer sessions were lost per restart).

Impact

High — users lose conversation context and history, which is disruptive to ongoing work.
