[BUG] Recent activity shows the same session multiple times

What's Wrong?

The "Recent activity" section on the welcome screen shows the same session multiple times. A single session appeared 3 times with different timestamps (12m ago, 11h ago, 11h ago).

What Should Happen?

Each session should appear exactly once in the Recent activity list.

Error Messages/Logs

(no error - just visual duplication)

Steps to Reproduce

1. Start Claude Code
2. Observe the "Recent activity" section on the welcome screen
3. The same session appears multiple times with different timestamps

Note: I started only one session with this message, but it appeared 3 times.
