[BUG] Recent Activity not consistently shown on startup

What's Wrong?

When launching Claude Code from a given directory, it doesn't show "recent activity" the majority of the time, seemingly at random.

What Should Happen?

Recent activity should be populated.

Steps to Reproduce

1. Launch Claude in folder X, and type a few messages
2. Repeat step 1 two times
3. Open Claude a fourth time, and see no recent activity
4. ???
5. Open it an Nth time, and see your recent activity suddenly appear, only to open it an N+1th time, and see nothing again
