[Feature Request] Reduce prompts for user feedback on agent performance

Feature Request
Feature: I tell you when it is working badly, instead of you asking me all the time.

By asking at random you end up asking me how it's going, at the time when it's going okay and _not_ asking when it isn't, because going okay is most of the time.

I'd rather just be able to send you that feedback when it's going badly so the absence of a report indicates all is well.
