[BUG] Session name changes do not sync between macOS desktop app and Android app

What's Wrong?

When changing a session name in either the macOS desktop application or the Android application, the updated name is not reflected in the remote environment of the other device. The session content syncs, but the session title/name remains unchanged on the peer device.

What Should Happen?

Environment
Device 1 (Desktop): macOS/ Claude Desktop App Version
Device 2 (Mobile): Android/ Claude Android App

Connection: Both devices are logged into the same account.

Steps to Reproduce
Open the Claude app on both macOS and Android.

Create a new session or open an existing one.

Rename the session on the macOS desktop app (e.g., change "New Chat" to "Project A").

Check the session list on the Android app.

Expected Behavior
The session name should update to "Project A" on the Android app via remote synchronization.

Actual Behavior
The session name remains "New Chat" (or the previous name) on the Android app, even after refreshing. The same issue occurs in reverse when renaming from the Android app.
