Image/screenshot attachments not delivered to the model (Claude Code on claude.ai)

Environment: Claude Code functionality on claude.ai chat, code mode

Bug: Screenshots/images attached by the user in the chat are NOT reaching the model. The model receives only the text of each message, with zero image data.

Details:

• Confirmed repeatedly during a long session on 2026-06-05: the model explicitly never received any of the many images the user sent (dozens that evening).
• Earlier the model correctly understood screenshots, so this looks like a regression that started that evening.
• Exiting and re-entering the chat does NOT fix it.
• The user could not be told in real time because the model had no signal that images were missing (it just received text-only messages).

Impact: High. Visual/UI debugging becomes impossible and causes large time loss and frustration, since the user reasonably assumes the model sees what they send.

Request: Please investigate the image-forwarding pipeline for Claude Code sessions (attachments uploaded by the user not being delivered to the model).
