Session sync between Desktop app and VS Code/Cursor extension

Summary

Sessions created in the Claude Code VS Code/Cursor extension are not visible in the Claude Code desktop app sidebar, and vice versa. The extension can see desktop sessions, but the desktop app cannot see extension sessions.

Expected behavior

All Claude Code sessions for the same project directory should be visible across all clients (desktop app, VS Code extension, Cursor extension, CLI).

Current behavior

Each client maintains its own session list independently. This makes it hard to continue work started in one client from another.
