[BUG] Any action in Claude Code opens the terminal in VSCode when linked through /ide

What's Wrong?
If I run Claude Code in iTerm outside of VSCode's terminal it will open the VSCode terminal window any time an event happens in Claude Code.
For example, the Claude Code: Insert @-Mention Reference command in VScode will add the mention to the Claude Code terminal interface but also open the terminal window in VSCode even though Claude Code isn't running there.
What Should Happen?
It should leave the open state of the VSCode terminal unaffected.
