[Bug] Claude Sonnet ignores user instructions and makes unauthorized code changes

Bug Description
Sonnet 4.6 continues to go rogue and make changes even when not instructed to. It has gotten worse in the past few weeks. The AI is making too many assumptions and not working as a team-mate
