[BUG] - Planning mode proceeds without final confirmation

What's Wrong?

This has been happening recently, as of the latest version, and it’s really starting to frustrate me. If the planning mode requires confirmation on certain questions, the expected behavior is that once you answer those questions, it finalizes the plan, shows you the plan, and then asks, “Are you ready to proceed?”

However, I don’t know if it’s because of the new auto mode or something else, but now, after asking those questions, if I step away and come back, I find that it has taken unauthorized action and gone straight into implementing the plan. This has already occurred a half a dozen times this morning. Planning should NEVER be allowed to move to implementation without explicit approval (unless otherwise overridden by some kind of YOLO flag).

What Should Happen?

Claude Should have shown the location of the actual markdown plan and waited for final confirmation before proceeding to implementation.
