[BUG] terminal title "icons" no longer seem to accurately reflect status

What's Wrong?

I believe the titles used to transition to the large *-ish thing when waiting for user input (ie in between turns, but also permission prompts/etc). I would use this to determine when (and which) needed attention. They seem now to only transition in between turns, so there isn't a visual signal at all (it instead continues to be the small centered dot moving back and forth).

What Should Happen?

I believe that the icon next to the title should transition to the *-ish symbol when waiting for user input (as it used to), or at least transition to SOMETHING visually distinct from the normal turn in progress, nothing-to-see-here state.

Steps to Reproduce

Set a policy that will result in a permission question and then prompt claude in a way that this will be hit. Title will remain unchanged.
