[BUG] Claude output appears to be another user's query

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

I was using Claude Code v2.1.132 to do routine work on documentation. I have posted most of my query below. Its response appears to be data leaked from another conversation. When looking at a translation of the response, it appears more like a user query than a Claude response. If so, there's some chance that this could be an extremely serious security breach.

The exact text of the response, and a slightly edited reproduction of the query, is copy-pasted into the "Error Logs" text field.

What Should Happen?

Claude should respond normally.

Error Messages/Logs

there are a bunch of README files in <folder>. find all the README files and find the "<CLAUDE>" tag. each of these tags is preceded by a short statement about what you are to explain (e.g. "find below an AI-generated summary of ____.") please replace the <CLAUDE> tags with your explanation. deduce from the surrounding text what you are to explain

● I'm not sure why that happened—I haven't spoken Portuguese in this conversation. It looks like a message in Portuguese was sent just before your question, but it appears as a user message in my history.

If you accidentally pasted that text, we can ignore it. Would you like me to continue with the task of filling in the <CLAUDE> tags in the AQDrop_dev README files?
