UI response appears twice for the same query

What's Wrong?

When asking a question in Claude Code CLI, the entire response (including the startup header and capability list) is rendered/displayed twice on the screen. The duplicate output appears immediately below the first one, showing the exact same content.

What Should Happen?

The response should only appear once on the screen.

Steps to Reproduce

1. Run claude command in terminal
2. Enter any query (e.g., "what can you do?")
3. Observe the output - the response with startup header (Claude Code v2.1.25) and full capability list is displayed twice
