[Bug] Ultrareview crashes before producing findings, incorrectly consuming free attempt quota
Bug Description
Ultrareview was launched on my branch safety/rescue-uncommitted-2026-05-15 →
main and crashed before producing any findings. The CLI reported it as "Free
ultrareview 2 of 3," meaning a free attempt was consumed despite the service
returning no usable output.

Scope at launch: 71 files changed, 11,486 insertions, 9 deletions.
What I'm asking for:
A refund of the free attempt that was burned on this crash. Given that this is
the second free attempt I've used and I received zero value from this one —
the failure was entirely on your infrastructure, not user error — I'd also ask
you to consider restoring the first free attempt as a goodwill gesture, since
I've now spent 2 of 3 free runs and only received findings from one.
Charging a user for a crashed run is a bad pattern. The free tier exists so
people can try the feature; a crashed run isn't a trial, it's a broken
product.
Please confirm the refund and, if possible, share what caused the crash so I
know whether retrying is worth it.
