[Bug] /ultrareview crashes without producing findings, consuming free quota

Bug Description
/ultrareview crashed twice without producing findings — both consumed my free quota. Branch: feat/pepa-post-v1 (Pepa repo) Date: 2026-04-27 Sequence: 1. First /ultrareview attempt: cancelled before launch (showed "Ultrareview cancelled.") 2. Second /ultrareview attempt: launched successfully ("Free ultrareview 2 of 3" — implying 1 prior consumed). 3. Received TWO task-completed notifications, both with payload: {"error":"Review crashed before producing findings. See session logs for details."} Task IDs of the crashed reviews: - rcf4btyhm - r6aar29ft Session URL shown at launch: https://claude.ai/code/session_01GmPQp6dMuQRyymHDDdgjsF Result: 2 free reviews consumed, 0 findings delivered.
