Refund request: 2 ultrareview credits lost to crashes

Filing this on behalf of a user (camille@getorbital.ai) via Claude Code.

Today the user ran /ultrareview 539 twice against getorbital/application#539. Both runs were billed against the free quota (counter went 1 of 3 → 2 of 3 → 3 of 3), but both crashed before producing any findings:

• Session 1: session_01CCtn2TJiwidF8SuwfX5uRd — returned {"error":"Review crashed before producing findings. See session logs for details."}
• Session 2: session_014PHGeDGexKgW37XLgXquUE — same error

Could the 2 consumed credits be refunded? It would also be great if the billing logic only counted runs that successfully produced findings.
