[BUG] /ultrareview consumes free credits on cloud-side crashes

What's Wrong?

Bug: /ultrareview consumes free credits on cloud-side crashes

Two consecutive /ultrareview runs crashed before producing findings,
each consuming one of my 3 free credits.

Failed sessions on PR drtylerblack-rgb/superego#25:

  • session_01Qqd25XEU6458QT7bUW2kUf — "Review crashed before producing findings"
  • session_019S8xCqsxEgWzbZc5KKzbdc — "API Error: Server is temporarily

limiting requests (not your usage limit) · Rate limited"

Both finished status was "completed" with the crash captured as the result.
No actual review output was delivered, but the credit was decremented at launch.

Request: refund credits for runs that crash before producing findings,
and/or scope rate-limit failures specifically as non-billable since they're
server-side capacity issues, not user-driven retries.

What Should Happen?

Claude should not burn credits/charges on server-side failures. That's extremely unfair.
